Everything You Need to Know About CCAH

Members generally pay an enrollment fee and monthly fee in exchange for personalized care coordination, wellness services, and guaranteed access to home-based support as needs change. Care is delivered at home whenever possible.

Most programs are designed for healthy, independent adults typically aged 60 or older. A health screening is usually part of the application process to ensure you can benefit fully from the program.

Services often include care coordination, wellness planning, home safety assessments, personal care aides, nursing support, transportation, and access to skilled care if needed. Coverage varies by program.

Unlike private care, a CCaH program includes ongoing care coordination, a predictable cost structure, and long-term planning. It offers more comprehensive support than traditional insurance and relieves your family from managing care logistics.

CCaH programs are designed to grow with you. As your needs change, your care plan is adjusted and additional services—such as in-home care or nursing—may be provided through the program, depending on your contract.

Yes. Your care coordinator/navigator can arrange for in-home services, therapy, equipment, or follow-up care, ensuring a smooth recovery and avoiding unnecessary transitions to institutional care.

Most programs require a one-time enrollment fee and a monthly fee, which vary based on your age, health, and selected service package. Many members find it to be a cost-effective alternative to long-term care insurance or facility-based care.

Absolutely. The program provides peace of mind for family members by handling care coordination, reducing the burden of caregiving, and offering support resources when needed.

Look for a program with a strong track record, a clear plan for care coordination, transparency about costs, and a compassionate team. Consider scheduling a consultation or attending an information session to learn more.

A CCaH program provides many of the same services as a Continuing Care Retirement Community (CCRC)—like long-term care coordination and guaranteed access to care—but allows you to stay in your own home. Unlike a CCRC, there’s no need to relocate, making it an ideal choice for those who want security without giving up their independence or familiar surroundings.
